Policies
- Licenses
- I am fully licensed in both Virginia and North Carolina.
- I have a DEA license for Virginia only.
- I carry my own insurance and liability.
- Contracts are made between my PLLC and your clinic.
- Fees
- Please contact me for current rates.
- Cancellation
- Please see contract details. Cancellations prior to 30 days from scheduled date of work incur no cancellation fees.
- Payment
- I will provide you with an invoice for hours worked every 14 days.
- Payment is due 14 days after the invoice date.
- Failure to provide payment or ineffective payment will incur late payment fees.
- Shift Expectations
- All shifts will be exam shifts unless otherwise specified in the written contract.
- For our mutual benefit, I do not perform soft tissue, orthopedic, or dental procedures.
- If given the proper equipment, I will perform sedated procedures, wound treatments, and laceration repairs.
- Clinics can expect professionally dressed, respectful, knowledgeable, and friendly service from me.
- I will provide all appropriate licensing information prior to the start of the first shift.
